Service Description
What can I expect in my 90-minute Initial Consultation? • Your case history will be taken. You will be asked various questions about your health, lifestyle and diet • Please bring copies of any recent medical tests for review • Via email you will be provided a 3-day food diary to fill out and submit prior to your appointment • A list of all current prescribed medications and supplements will need to be provided before your appointment • Please arrive 15-minutes early • Any health information taken will be kept secure and confidential Your case history will be utilised to build a personalised holistic nutrition plan. • If supplements are recommended, links to where these can be ordered will be supplied to you. If you need an online appointment instead of in person just ask and we will arrange it for you. Follow up appointments can be booked at the end of each consultation priced at $160.00 (in person or online available)
Cancellation Policy
I have been informed of the fees for service priced at $200 for a 90-minute initial consultation, and $160 for 30 minutes follow-up consultation, and I understand that payment is due when the services are provided. If I do not cancel an appointment at least 24 hours in advance, then I am liable for the fee, the fee being half of the consultation fee that I am currently booked in for.
